Skilled Nursing
Registered Nurses (RNs) and Licensed Vocational Nurses (LVNs) work with your doctor(s) to provide assessment, skilled treatment and support in home. Our skilled nursing staff can provide:
- Skilled nursing assessments
- Vital signs monitoring
- Diabetes Mellitus care
- Catheter care
- Ostomy care
- Injections and Intravenous fluid/medication management
- Educating patients and caregivers/family about safety, medication, diet/nutrition and disease.
Therapy Services
Physical Therapy , Occupational Therapy and Speech Therapy
- Physical Therapy: our physical therapists will come to your home to assess and treat functional impairments that result from injury, falls, stroke, surgery, fractures, or other medical conditions which pertain to your lower body such as lower back and legs. Therapy services include exercises, gait training and instruction for the proper use of durable medical equipment such as walkers and canes, pain management, home and mobility safety precautionary education for patients and caregivers.
- Occupational Therapy: our occupational therapists will assess and treat functional impairments which pertain to the upper body such as the shoulders, neck, arms and hands.
- Speech Therapy: our speech therapists will come to your home to assess and treat patients who suffer from speech impairments and difficulty speaking/communicating or have difficulty swallowing or have dysphagia.
Other Services
Certified Home Health Aides:
Home health aides are certified nursing assistants who can provide patients with assistance for: personal hygiene, bathing, ambulation and light household tasks.
Medical Social Work:
Our medical social workers can help provide guidance, education and assistance including: coordination of benefits and community programs available to patients, assistance in financial matters, housing and caregiving services, home safety evaluation and family counseling.

Available with referral from your primary care physician or facility.
